9:00-Noon   Living With and Through Loss: The Epic Journey of a Lifetime

Abstract: Loss is an inevitable part of the human experience, extending far beyond bereavement. This session invites participants to reflect on the wide spectrum of losses we encounter across our lifespans—relationships, identity, health, opportunities, and more. Through shared readings, film clips, and music, attendees will explore the universal and personal dimensions of grief, mourning, and resilience with the goal of leaving this workshop with deeper insights and tools to support  their patients and families through loss and also navigate their own experiences.

12:30 - 3:30     The Ethics and Art of Chart Noting
                          Laura Guidry-Grimes, PhD, HEC-C

Abstract: The medical chart documents a particular story about the patient and their family, and the chart note author makes numerous decisions and value judgments in the process of prioritizing (and deprioritizing) certain information and choosing some language over others. The chart note is a powerful communication tool, and especially since the 21st Century Cures Act, it has important implications for the therapeutic relationship with the patient. This workshop offers insights and techniques for minimizing bias, addressing patient vulnerability, and building a stronger therapeutic alliance through chart noting. Attendees will have time to discuss their own chart noting challenges and collaboratively problem-solve in groups.  Deidentified chart note examples will also be made available for groups to analyze and improve upon with feedback from the facilitator and others.
